Tier 1 and Tier 2 provision

On behalf of Healthy Lives Together, North East Lincolnshire CCG hereby invites applications from suitably qualified and experienced providers to provide a Social Prescribing Service in North East Lincolnshire. These will be made up of one Tier 1 Provider and several Tier 2 Providers.

The Five Year Forward View (FYFV, NHS England, 2014) specifically highlighted the need for new approaches to health care provision that utilise and combine with community assets. The Social Prescription model fits with the NHS FYFV and provides an effective means to address these support factors.

It is widely understood that wider social, economic and environmental factors have a significant influence on people’s health and wellbeing.

Social prescribing can help address some of these wider determinants of health by linking patients in primary care with sources of support in the community.

Within the local context Social Prescribing forms part of the CCG’s commitment to reducing pressure on the health and social care system and dovetails with other local services providing prevention-focused services and activities.

It should be noted that this Social Prescribing services are not commissioned by North East Lincolnshire Clinical Commissioning Group (CCG). It is being commissioned by Healthy Lives Together who will contract directly with the Tier 1 and 2 Providers.
As this will not be a Public Contract this procurement does not fall under the Public Contract Regulations 2015
