About the CCG
We are an award-winning group of 26 GP practices in Grimsby, Cleethorpes and Immingham working together to improve the health of local people.
Our CCG is responsible for commissioning health and adult social care services for over 165,000 people in North East Lincolnshire and we’re committed to putting families and communities at the very heart of everything we do.
We are part of the Humber Coast and Vale Sustainability and Transformation Partnership and part of the Humber Transforming Care Partnership for Learning Disability.
Our organisation is made up of people who understand patients’ needs best.
Doctors, nurses and health professionals from 30 GP member practices work with a forum of volunteers from the local community so the people of North East Lincolnshire receive high-quality care when they need it.
The CCG has delegated Primary Care Commissioning arrangements with NHS England.
By working in partnership with North East Lincolnshire Council we’re able to deliver joined-up health and adult social care services for local people.
Historically our health and adult social care budgets were combined and we are continuing to work in this way because it allows us to provide more seamless care for patients.
We were established in April 2013 following changes to the Health and Social Care Act 2012 and replaced the former Care Trust Plus.
