Description of Service:

Social Prescribing

Supporting patients living with the following long term health conditions, aged between 18-65 and living in North East Lincolnshire.

  • COPD
  • Asthma
  • Diabetes
  • Hypertension
  • Atrial Fibrillation

The Service can help with the following:

Our aim is to support individuals who are living with one or more of the above long term health conditions. Patients are assigned a link worker who will work with them to create an action plan and help them to identify other issues that may be impacting on their health and wellbeing. We are able to support patients over a 2 year period to engage in and help sustain lifestyle changes. Support and advice can be provided regarding benefit, debt and housing issues. Patients can also take part in community based activities that can really make a difference such as weight loss programmes, advice on healthy eating, social activities, gym, swimming, chair based exercise, gardening, support groups, bingo, tai-chi, library access, sewing classes, IT skills and confidence building.
