NELCCG COPD (Chronic Obstructive Pulmonary Disease) Patient Survey, COPD Social Prescribing Engagement Event & COPD Patient Focus Group
North East Lincolnshire Clinical Commissioning Group is conducting a COPD (Chronic Obstructive Pulmonary Disease) Patient Survey and we would like your help to gain views on support which will help manage COPD. All feedback will help us to see if any changes are needed and what these might be.
Please can you pass on/forward this survey on to anyone you know who is affected by COPD.
You can return completed paper surveys to our Freepost postal address. You can find details of this on the survey.
The survey is also available to all COPD patients to complete online using the following link:

The electronic/postal survey closes on 30th April 2016.
We are also conducting a COPD Social Prescribing Engagement event in April & a COPD Patient Focus Group in May 2016 so again please pass this information to anyone you may think would benefit from attending.
